This could get very long and I don’t want to do that at the moment, so I will sum it up by saying that my pcp is brand new to practicing and has not been much help at all and so I have been doing much of the research and legwork myself. Symptoms started around a year and a half ago. New symptoms keep piling up like Tetris. My hunch was kidney issues or MM. Blood in urine (dried blood flakes even at times), painful ribs (albeit point/touch specific), recurrent UTIs that no longer respond to drugs, petechiae that is now spreading, random fevers with no cause that last a few hours, swelling, peripheral neuropathy in both of my hands all day every day, exhaustion, etc etc. I recently (October) had a UPEP (24 hour) and my pcp never said anything was off about. I just yesterday was looking into it and it seems from reading that it may have been very abnormal? That, on top of my last 2 SPEPs (just me trying to get some kind of resolution) having slightly raised kappa light chains (normal others) and slightly low IgG, I am now wondering about the possibility of MM without a M spike? I see that I have many globulins (even as high as 20%+ of Alpha-2) but it also says in the note that no M spike was seen. Right?

Been to two urologists and a nephrologist and none thought my kidneys were struggling bc my GFR was normal (despite a downward trend of GFR and other things). I DO have another hem/onc referral scheduled for end of Jan. First hem/onc I saw in May (before these SPEP and UPEP were done) told me that the teardrop cells I had in my manual smear (which pcp referred me for) were “sometimes normal” and she ran a CBC and told me I was fine. She also wasn’t concerned about my immature granulocytes being high consistently for over a year as well as low mpv consistently for over a year. Just basically looking for info on these reports and trying to learn what I can so as to know if I need to advocate more with my pcp.
