Now a tear is a tear and well I learned the hard way . I have a musy tendon no one seen till the operation well it tore again as the MRI did not even pick up the tear . I know and MRI reader is just that a reader and well mine was missed over and over . The Rule my ortho lives buy is this if the shots are not working and pt is not working they have no choice but to fix it. So no it does not have to be torn 50% It just has to be hurting and your doctor to avoid a lawsuit must do all he can before surgery at his comfort zone. If you feel he is not helping you and you have tried all you can to make your body heal on it owne and nothing you need the surgery. But let me tell you and you will see others that have had this surgery some do good real good and I did not my tendon broke again and is now going to the other shoulder I hurt it again in PT after surgery . There was nothing left of the tendon and the MRI did not see it only the pain from it. This surgery is very painful I have had other surgery in areas and none was a tuff as this one and I still suffer with different pain now related to it and now more tears So my tendon was torn and not healthy . If you want this surgery it is your choice and change to a dr who will listen. Now other factors make mine worse smoking , and a +ANA I will stay inflamed and this is not good . Hope that helps its your life and let your dr know you have had enough and would like the surgery if you have tried all and have failed. Hope that helps

I do not have an answer for you but am curious about the question. I have been told that I need surgery for a massive rotator cuff tear. Do you have experience with this type of surgery?
