Worried mommy: is it normal for 1 year old to spit up once every 2-3 days?

My baby was born at 27 weeks. He is very healthy, happy, and active, just a week short of one year (adjusted age). No health problems. He weighs about 23 lbs. When he was little, he arched his back a lot after his feedings. We consulted the doc and the doc said he's likely to have reflux but he's gaining weight fine and would outgrow it so he didn't think it's necessary to get any tests done. My baby spit up projectile once every 2-3 weeks so we weren't too concerned. It got less frequent as he got older. However, he started to spit up once every 2-3 days. He falls asleep right after his last bottle (7 oz) and wakes up one hour later screeming. I hear hard swallows and he has burps coming out. He would start coughing and then spitting up his feeding. I'm not worried about his weight gain but he looks so upset and cries so hard. Is it reflux? Am I overfeeding him? Is the new texture confusing him? I'm giving him stage 3 baby food and sometimes our food in small pieces.
I'm wondering if other moms have experiences. I would really appreciate your kind suggestions.

From my experience with 30 weeker twins, they went through a stage when their reflux subsided, but picked up again, right around your son's age. The good news was, it went away for good by 16 months. A few things are possible culprits. If he is going through a teething bout, it could be irritating his reflux. He could just be getting used to the new foods you are introducing. And, he may just have a lot of liquid in his system, is very active, and is spitting up.

How much are you feeding him? By the time my girls were one year adjusted, they were only on table foods, and some stage 3 foods. I realize some parents take it slower, but I think it's totally normal to begin to reduce the liquid intake and increase the solid intake by one year adjusted, per my pediatrician. He told us after one year (we switched to whole milk, I realize some switch later on) that they should be eating 16 ounces maximum of milk a day, and 4-6 ounces of solids per meal, with two 2-3 ounce snacks between meals. So, they were essentially getting 16 ounces of milk, 6 ounces of water (at snack times) and 16-20 ounces of solids a day. I doubt you are overfeeding him, b/c my friends tell me that this is a pretty hefty food volume. My girls are good eaters.

Is he doing 3 meals a day, with 2 snacks and 2 naps? My pediatrician recommended (by one year) not giving bottles before bedtime or naptime, so as to not upset the tummy and not associate mealtime with eating. For us, a schedule worked great, with 3 meals, (7am, noon, 5pm) and naps at 9 and 1, with a snack right after the naps. Their reflux was better on this type of a schedule.

That sounds very similar to what my son has done... we have even heard the burps come up through his body and then get immediately followed by vomit. My son is 20 months and we give him a 7-8oz bottle right before bed. We believe it is due to air in his tummy. My son has a g-tube, so we are able to vent him a couple of times during this last bottle of the day. Sometimes there is a lot of air, other times none. Since we've started venting him frequently during this last feeding, we have been able to avoid most vomits and he sleeps better. If your son doesn't have a g-tube then I'd recommend frequent burping. My son also has severe reflux for which we have recently increased his dosage of Prevacid. This may also have helped him.

I agree with the previous posters. . it does sound like reflux or maybe just a little too full before laying down for the night. We always called those burps that ended with them spewing "burps gone bad" LOL! Extra burping or more vertical time after the last bottle might help too. It doesn't sound bad enough to require meds, but after adjusting his eating schedule and trying some other tricks, if it's not better, maybe see what his pediatrician thinks.
Our 30 weeker, Ally, now almost three and a half years old is on Prevacid for terrible reflux that seemed to actually get worse as she got older beginning at about 18 months old. Before we started her on the Prevacid, she would 'spit up' . .literally just a TB or two while she was playing. . I know some people gave us strange looks at times thinking 'babies spit up. . not 2+ year olds' LOL! You should see what she can do if we forget her medicine! Then it's really a show!
Oh. .what would we do without these little ones keeping us on our toes? :)
Good luck! Hopefully making a few adjustments will help him sleep better and not spit up anymore.

DS is 25 months old; he is a former 32 weeker. He still randomley pukes from reflux. He was on prevacid until he was 22 months old. For now he doesnt need to be on meds so we just deal with the random puking. I would say its a normal reflux thing.
