my son daniel was born 2 1/2 months early and stayed in NICU for 2 months. During his stay he had 2 blood transfusions for anemia. His counts were 22 and then later 24. I know after they get a transfusion their counts go way up and then they eventually go down before gradually coming back up again. I've had him home for 1 month now and lately he's been struggling with his feeds. He has acid reflux so I don't know if it's due to that because he has been on the same dose and is now 2 lbs bigger then when I first brought him home. He breathes really loud after he eats and he's been spitting up a lot. He's been averaging 2 ounce every 3 hours but he weighs 9 lbs. last week he was able to take 3 oz every 3 hours. Anyone have any advice?

I would start by talking to your ped about his reflux med dosage, especially if he isn't pale, inactive and only exhibits symptoms during or after eating. My son went thru something similiar abouth a month or two after he came home. He gained weight pretty quickly for those first few months that the dose on his reflux meds was not high enough and his reflux was bothering him. It may sound strange but even a pound in babies can be a big difference when it comes to their dosages.

I agree. He sounds like my son and I would definitely have the reflux meds evaluated and you might want to do the dysphagia study to make sure he does not aspirate while eating. It is easy and quick to do and not hard on the kids. Med's for reflux constantly need to be monitored as they gain a lot in the beginning.

Could he have a food sensitivity to what he is eating? I ask b/c my son has had sensitivities to both cow's milk (e.g., in my breast milk and from fortifiers made from cow's milk) and soy (again either in my milk or in formulas). When he was 9 months adjusted, he even started being sensitive about his elemental formula (I wasn't producing enough breast milk, so we have had to supplement some bottles). At that point, we tried goat's milk, which for whatever reasons was well tolerated.

At 5 months adjusted, we did have to up his dose on his reflux medications b/c he'd gained quite a bit since he was brought home and thus needed something stronger. Also, he was initially on the generics of Pepcid and Zantac, which didn't work for him. Around 1 month adjusted, we tried Prevacid, which has worked for him (but again, needed to be upped on the dosage when he got heavier).
