I am 38, and have been diagnosed since the age of 30 with Osteoporosis. I have been on Calcium/vitamins supplements as well as hormone replacement therapy (the reason for my early osteporosis)....my doctors have been hesitant to use Fosamax because of lack of long term study---potentially, I could be taking this for 20++ years. Recently, I went off the hormone replacement, and my bone density shot way down (-3.2). Now, back on hormone replacement, calcium and vitamin supplements but wondering if anyone else with same scenario or advice. Both my OBGYN and Endrocronologist have never seen such bad scores in someone so young, and I dont think they really know what to do. I do not have any bone pain, or issues with fractures. I want to make good, long term decisions. Any advice?

I would ask to have the 24 hour urine and a blood test to see what your Vit. D level is at and how much calcium your body is getting rid of. If the Vit D. is low that could be the problem. You need Vit D to help the body absorb the calcium. Perhaps you have had that done already and just did not mention it. If not, it is important to do so.

I was diagnosed at the age of 33 and have been on Fosamax for 12 years now. My scores have gone from -2.5 to -1.2 this January. My endocrinologist said it has been on the market the longest and studied the longest. But I'm like you - I will be taking it for many years and not sure what the long-term results will be.

Hi Pacer: I was dx'd at 30, but not with a t-score. I had a bone biopsy that showed osteoporosis. When I finally had a dxa, years later my score was -3.6.

Unfortunately no one can tell you what will work for you, but having a drop in score from loosing hormones is the norm. I went through early non surgical meno and wasn't put on HRT, so the general consensus was my bone declined as a result of that plus family history of osteo and other spinal probs.

During the years from dx to my first dxa I took supplements and exercised, but for me I didn't see an increase in scores until I took Forteo. I'm almost finished with this med and have improved a whole point. I started out on Actonel, but was switched to Forteo when my scores didn't improve.

I know lots of people that improve on HRT and supplements, bisphosphonates and other meds, so it's something that you'll have to decide on after determininig which is best for you.

I agree on the 24 hr urine test and some others like thyroid, parathyroid, and vit D tests to see if you have a secondary cause for this, which can be treated, but in a different way than with bone meds.
