Mediastinoscopy

My mom was diagnosed at 3a adenocarcinoma in August 08. She finished 7 rounds of Taxol/Carboplatin and 25 daily radiation about 3 weeks ago. My mom had another CT scan about 1 week after her chemoradiation therapy. The new CT showed 90% reduction in the main tumor. The lymph node in her middle chest has reduced from 2.1 cm. The CT report said that the node is now mildely enlarged. We just met with the surgeon today, and he wanted to do a mediastinoscopy to check her lymph node next week. If it is positive, then he would not consider surgery. If it is negative, then he will do the surgery to remove her right upper lobe. Does anybody have something similar? Could the enlarged lymph node cause by inflammation due to the radiation? I am really worry that the lymph is still postive because the surgeon is suspecting the same way. Since her main tumor has reduced 90%, wouldn't the chemo and radiation take care of the lymph node as well? My mom did the CT scan about 1 week after her induction treatment, it is now three weeks after her treatment, would the tumor continue to shrink? Sorry about all these questions. I guess I am really trying to vent.

Hi K, i'm an 3A nsclc 9yr survivor. I had surgery after chemo and radiation. They removed my two lower lobes of my rt lung and the lymph nodes in the center of my chest. One lymph node had cancer cells but they were dead from the radiation. No one ever knew it was affected. I have since found out that they would have followed what your mom is going to do. All I can say is one day at a time. Stay possitive and be strong for your mom. God bless and good luck. Keep us informed. We'll be here for you when you need us. 24/7- John

My situation was the same . I also live in So. CA. I am 111A & had chemo, followed by surgery (LUL) & some lymph nodes in the mediastinum. Why won't they operate if there is lymph node involvement? I had more chemo & Rad. AFTER surgery. (Cause surgeon recommended NOT to radiate first...) Maybe you can get a second opinion from my Surgeon. He's at Cedars. I'd travel around the world to go to this man! Betsy

Thank you John for your support. My mom is going to City of Hope right now. What is your surgeon's name, Betsy? The surgeon explained that based on the clinical trial, if her mediastinum still involved after chemoradiation, they might suspect that she will have microscopic cancer all over her body. By doing the surgery would not increase her survival rate. I really want her to have the surgery. Her 1st oncologist (not at City of Hope) think surgery is possible and he plans on giving her more chemo after the surgery. If City of Hope turns her down, we might get another opinion from her first oncologist and go back to the surgeon he recommended the first place. Betsy, was your lymph node involved before the surgery?
