Physical forms for school activities

My daughter wants to try out for the school Cheer squad. This being the first activity since her dx and treatment I found myself really having to think about how to condense her medical issues without taking up the whole page! She is in remission so I just wrote "history of cancer" on everything....Should I have to give more than that?
(she finished trtmt. July 09 and is taking NO meds currently) It was just really odd for me for some reason.

That is what I fill in on the T'ball and soccer forms. My son is 61/2 so either myself or his father is always there at functions. But to be honest you might be shocked at the lack of interest or see if anyone even notices when the forms are reviewed. The mountain of school forms that I filled out and explained and attached extra notes to were apparently un noticed until the principle called me in to the office to complain about his tardies. ( 5-10) minutes 1-2 times a month when he felt nauseated in the morning. They were a little more than embaressed I think. I was not to say the least.
I'm so happy that she's well enough to take cheering on. Congrats!!! ENJOY mom!
